You can contact the Public Relations Office of the Fondazione Toscana Gabriele Monasterio to submit observations, comments, suggestions or complaints.

Functions of the Public Relations Office

The Public Relations Office is the reference point for your relationship with the Monasterio Foundation.

It receives observations, suggestions, comments and complaints in any form submitted by the patient, his or her relatives or relatives-in-law, voluntary organisations or rights protection organisations accredited by the relevant region.
The Office provides, on behalf of the Managing Director, an immediate response to the user for reports that are predictable, unambiguous and certain:

It prepares the preliminary activity, acquiring all the elements necessary for the formation of judgements (reports or opinions) from the heads of the Units and the offices concerned, and advises the General Manager on those complaints that do not lend themselves to immediate and rapid settlement;

It prepares the response letter to the user, signed by the Managing Director, guaranteeing a response within 30 days;

It activates the complaint review procedure before the Ombudsman (regional, municipal or consortium) or another “super partes” figure, if the user is dissatisfied with the results produced in the first instance.

Requests for a copy of the health record

The patient, or caregiver, who wishes to do so may request the Office of Public Relations to copy, in full or in part, the Medical Record, the Radiological Examination, and other Health Documentation.

The request can be made in person (in one of the hospitals of Monasterio), by email or by Certified E-mail.

The request consists of filling in a form and paying a fee, as set out in the attached table.

This, instead, is the form to delegate another person to the withdrawal of a copy of Medical Records, Radiological Examination, and other Health Documentation.

The patient may request to receive a copy of the health documentation:

  • by hand
  • by email (encrypted)
  • by Certified Electronic Mail
  • by registered mail
